Mutual misconceptions between designers and operators of hazardous installations

چکیده
This document provides a guide to research that has been conducted on the misconceptions that designers of hazardous installations sometimes have about operators and the operating environment, and the misconceptions that operating and maintenance staff have about designers, the design process or design intentions. These misconceptions are central to many accidents and incidents, and it seems possible that knowledge about how past misconceptions have contributed to past failures could be used to forestall future failures. The first part of the document provides a simple process and some resources to apply this research. It helps operators test whether they have misconceptions about the design. And it helps designers test whether they have misconceptions about operations. The second part is a description of the research, the background, methods, results and conclusions. collaboration with a team at Loughborough University. The Role of Quantitative Risk Assessment in Improving Hazardous Installations Siting: A Case Study
Safety distance has already been a main measurement for the hazard control of chemical installations interpreted to mean providing space between the hazardous installation and different types of targets. But, the problem is how to determine the enough space. This study considers the application of quantitative risk assessment to evaluating a compressed natural gas station site and to identi...

متن کاملLand-use planning in the vicinity of major accident hazard installations in Greece.
Land-use planning, as concerns the prevention and limitation of the consequences of possible major accidents from industrial installations, is an essential mechanism for dealing with actual or potential conflicts between sources of risk, such as potentially hazardous industrial developments, and surrounding land-uses. The objective of this paper is to present a decision making methodology that ...
